Overall Purpose

P3 is seeking a Medical Records Clerk to assist with chart management as well as general administrative support. In this role, you will be responsible for 50% of your time on medical records and 50% administrative support, including:

  • Supporting the AZ operations market team
  • Managing the AZ market office
  • Respond to release of information (ROI) requests, logging all authorizations and releases. Other
  • Tasks may include scanning documents to update electronic health and medical records as well as requesting via phone, email and fax medical records from other entities for P3 patients.


All applicants should be familiar with HIPAA compliance requirements. Medical Records Clerks should be friendly, able to communicate and operate a computer, and they should have the following skills to handle the job effectively:

Essential Functions

  • Retrieves medical records from various hospitals Electronic Medical Records systems.
  • Submit medical records (manually or electronically) with claims per payer requirements.
  • Must maintain 85 % quality rate based on audit criteria. Achieves goals set forth by supervisor regarding error-free work, transactions, processes, and compliance requirements.
  • Reviews source documents to determine accuracy and completeness of information of data.
  • Establishes working relationships with billers, enrollment, coders, and client relations for the purpose of obtaining necessary information for a medical records request
  • Assists other accounts receivable team members as needed to meet departmental deadlines, including assisting with overloads.
  • Assists in maintaining patient information and data in accordance with HIPAA regulations.
  • Performs other duties / functions as may be assigned to the position.
  • File documents in patient electronic charts
  • Check charts for accuracy and completeness
  • Update patients' electronic health or medical records
  • Respond to ROI (release of information) requests
  • Follow all HIPAA regulations
  • Typing, transcribing, proofreading and editing of routine and non-routine correspondence, reports and memoranda
  • Screening telephone calls and visitors and providing assistance or resolving inquiries where possible
  • Schedules and maintains calendar of appointments, meetings and travel itineraries and coordinates related arrangements
  • Composes and/or initiates correspondence, memoranda and reports
  • Works on special projects, including meetings or conferences, activities, events, mailings, presentations, reports, etc. — some requiring research and compilation of data. May coordinate with others the work and completion of some projects
  • Responsible for quality and continuous improvement within the job scope
  • Maintains records of expenses and compiles expense reports
  • Contributes to and supports the corporation’s quality initiatives by encouraging team and individual contributions toward the corporation’s quality improvement efforts
  • Performs other work-related duties and responsibilities as directed, assigned or requested
  • Must be able to multitask and manage multiple schedules/calendars
  • Assist with other clerical tasks as needed


Knowledge, Skills, And Abilities

  • Proficiency in Microsoft software applications: Word, Excel, Powerpoint, Teams, Outlook
  • Strong interpersonal skills
  • Ability to multitask and manage time independently
  • Organizational skills a must


Experience

  • 2 plus years admin assistant or medical records experience


Education

  • Knowledge of medical terminology a plus
  • Basic computer skills
  • Attention to detail
  • High school diploma or equivalent
