About the Job

Waystone is looking for a compassionate and experienced Case Manager to join our Adult Foster Care (AFC) Team! In this role, you will provide case management to individuals with intellectual and developmental disabilities to help them remain in their homes. Your day will consist of home visits and working collaboratively with the AFC team and the individuals' formal and informal support system.

Location: Haverhill, MA, with travel within the Merrimack Valley, Lowell, North Shore, Central Middlesex and Metro North for home visits is a requirement.

Schedule: Monday – Friday varied hours to meet the needs of the families we serve.

Compensation range: This is a 40-hour, non-exempt full-time position. The pay range is $21.50 – 23.76 per hour. This position is benefit eligible as of date of hire.

Job Responsibilities

  • Conduct regular home visits with each individual at the qualified setting
  • Provide counseling and support for the family, monitor health and safety in the home, and regularly receive and review the AFC caregiver log
  • Participate in the development, implementation, and ongoing review of the AFC plan of care
  • Support, evaluate and train families and caregivers in conjunction with the registered nurse
  • Provide referrals to community service organizations when needed, assisting individuals and families to advocate for their needs and providing information and referral to community resources when required
  • Provide case management as needs arise and assist families with coordinating care and services
  • Document all visits and communication and complete required documentation. Conduct on-going assessments of the home, monitoring for conditions that affect the health and safety of the individual

Qualifications

  • Bachelor’s Degree in Psychology, Special Education, Child Development or related field with a minimum of two years’ experience in the care of older adults or disabled adults
  • Experience in working with families in their homes is preferred.
  • Candidates must be able to read, write, and comprehend English to create and implement the family support plan, document services provided, and communicate effectively with individuals, families, and healthcare professionals
  • Valid driver’s license in good standing with minimum of 1 year driving experience
  • Strong communication, organizational and time management skills
