Job Summary

The Triage Nurse collaborates with the health care team to provide patient care while maintaining high quality operations of the facilities. The Triage Nurse is responsible for the duties of their designated position, but as time allows, they are to assist the other nurses if needed in completing their tasks. The Triage Nurse’s task list includes, but is not limited to:

  • Screens all patient calls and responds to patient in a timely manner.
  • Conducts a focused assessment of the patient’s health status through collection of data.
  • Communicates/directs messages to RN or NP/PA/MD as appropriate.
  • Accurately documents all messages/conversations in patient’s EHR.
  • Reviews the patient’s chart and most recent visit notes to determine plan of care.
  • Reviews patient’s chart to determine appropriate patient-specific standing orders.
  • Provides patient education after RN or NP/PA/MD assessment.
  • Pends medication for providers to send.
  • Approves/denies/forwards medication refills.
  • Initiates prior authorizations, tracks progress of requests along with all related appeals, and notates in PM/EMR software.
  • Creates diagnostic orders, therapy orders, and procedure orders as directed by NP/PA/MD.
  • Makes contact with patients as needed and follows up on all incoming calls received prior to 4:00PM.
  • Assists other departments as needed, such as medication prior authorization.

Supervisory Responsibility

This position has no supervisory responsibilities.

Qualifications

  • LPN or RN licensure required.
  • Minimum of one year of strong customer/patient experience with an exceptional ability to listen and communicate with others.

  • Detail oriented and able to multi-task and manage priorities.
  • Organizational skills with focus on tracking patient care and improving patient flow.
  • Ability to work in a fast-paced work environment and efficiently handle daily fluctuation of patient volume.
  • Can work independently or as part of a team.
  • Self-motivated.
  • Excellent customer service skills.
  • Able to follow oral and written instructions.
  • Strong computer skills.

Work Environment

When performing the duties of this job, the employee regularly works in a clinic and office setting.

While performing the duties of this job, the employee is regularly required to stand; walk; sit; use hands to grasp, handle, or feel objects, tools, or controls; reach with hands and arms; balance; stoop, kneel, crouch; talk, hear and smell. The employee must be able to lift or move up to 10 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.

Compensation & Work Schedule

  • Full-time: 40 hours/week.
  • Non-Exempt.
  • Day Shift: Monday - Friday, 8:15AM - 4:45PM
  • No evenings or weekends. A competitive benefits package including medical, dental, vision, disability, and life insurance, 401(k) plan, PTO time, paid holidays, and HSA is offered.
