AuthoraCare Collective is currently seeking a Scheduler for our Burlington campus. This is a Full-time position Monday - Friday, 8:00am - 5:00pm with rotating schedule to 5:30. This is a hybrid position with orientation on-site in Burlington.

Job Summary

The non-clinical scheduler works directly with the Clinical Manager and Director to provide high quality clinical support to patients through scheduling all patients visits from admission to subsequent death, navigating the electronic medical record (EMR) system, and checking in patients via the EMR. Communicates effectively both verbally and in writing with various teams, patients, and families. Identify patient rescheduling needs and coordinates accordingly.

Requirements

Education and Experience:

  • Home Care Home Base experience is a plus!
  • High School diploma/GED or equivalent- Required.
  • Ability to establish and maintain effective working relationships with patients, families, interdisciplinary team members, and the public.
  • Microsoft Word, Excel experience preferred.
  • Proficiency in scheduling software
  • Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.

Key Responsibilities

  • Able to navigate or learn Home Care Home Base EMR and other applications.
  • Knowledgeable about medical terminology.
  • Demonstrates flexibility in a demanding work environment.
  • Must have excellent customer service skills.
  • Familiar with Microsoft Office products including: Word, Excel, TEAMS
  • Evaluates, determines, and coordinates discipline visit frequency and scheduling changes.
  • Schedules timely initial visit frequency and coordinates with referral intake.
  • Responsible for patient and employee visit schedules
  • Reports to Clinical manger/designee on productivity and organizational metrics
  • From general direction, provides clerical and general support and data entry
  • Assist with incoming calls (completes or directs them appropriately)
  • Routine phone calls to patients/caregivers and clinical staff to finalize patient schedules.
  • Employee may be required to perform other duties as assigned.
