The Licensed Practical Nurse (LPN/LPT/LVN) provides direct and indirect home care, following physician orders and care plans.
Documents patient care, health status, and communicates updates to families and physicians.
Administers medications, offers health education, and conducts assessments as needed.
Follows safety protocols, maintains patient confidentiality, and ensures regulatory compliance.
Collaborates with RN and team to plan and evaluate care, participating in staff meetings and training.
Handles medication management, documentation, and coordination of appointments.
Provides training for new staff, covers on-call hours, and performs other duties as assigned.
Qualifications include a nursing degree, current license, CPR certification, and preferred home health experience.
Offers flexible schedules, PTO, medical benefits, 401K, and a pay rate of $31/hour.
