Position Summary

The Patient Education and Enrollment Specialist serves as a key liaison between patients and Valley Healthcare System by educating patients on available healthcare programs and assisting with the enrollment process. This role includes eligibility verification, patient advocacy, and connecting patients with essential resources. The specialist also contributes to the overall patient experience by supporting outreach, encouraging patient engagement, and promoting wellness services.

Duties And Responsibilities

Patient Education & Advocacy

  • Educate patients about available healthcare programs, including eligibility criteria and enrollment processes.
  • Provide one-on-one assistance with applications for Medicaid, Medicare, disability benefits, and other community resources.
  • Promote patient understanding of the importance of healthcare engagement, such as annual wellness visits and chronic care participation.

Enrollment & Eligibility

  • Guide patients through enrollment in insurance programs, chronic care management (CCM), remote patient monitoring (RPM), and other health services.
  • Collect, verify, and submit required documentation to ensure timely and accurate enrollment.
  • Verify insurance eligibility and program qualifications using available databases or payer portals.

Scheduling & Coordination

  • Schedule patient appointments for annual wellness visits, dental screenings, vision screenings, CCM, and RPM services.
  • Coordinate follow-up care as needed to ensure patient compliance and continuity of care.

Data & Record Management

  • Maintain accurate records in the Electronic Health Record (EHR) system related to patient enrollment, services, and follow-up needs.
  • Document all patient interactions and services provided in compliance with organizational and regulatory standards.

Communication & Outreach

  • Serve as a liaison between patients, providers, and external organizations to resolve coverage issues and facilitate services.
  • Encourage and assist patients in leaving online reviews to boost engagement and clinic visibility.
  • Educate patients on the benefits of utilizing in-house pharmacy services.
  • Perform other duties as assigned.

Evaluation

Work performance, including duties, responsibilities, demeanor, and work ethic, will be evaluated on an ongoing basis by the Chief Financial Officer in accordance with center policies.

Requirements

  • High school diploma or equivalent required.
  • Minimum 2 years of experience in patient services, community health, insurance enrollment, or related healthcare field.
  • Working knowledge of Medicaid, Medicare, and public assistance programs.
  • Proficiency with electronic health record systems (e.g., eClinicalWorks) and standard office software.
  • Strong communication, organizational, and customer service skills.
  • Bilingual ability preferred, depending on service area needs.

Status & Compensation

  • Status: FLSA Non-exempt; OSHA High-risk; ADA Negotiable
  • Compliance: Must adhere to all company policies and Federal, State, and Local requirements, including HIPAA Privacy & Security regulations.
  • Confidentiality: All employees must sign and comply with the Pledge of Confidentiality regarding patient PHI (Personal Health Information).
  • Compensation: $15.96–$23.95 per hour, based on your knowledge, skills, and relevant experience.

Physical Requirements

  • Prolonged periods of sitting, standing, and walking.
  • Occasional travel for community outreach.
  • Ability to lift up to 25 pounds when setting up outreach events or transporting materials.

Environmental Working Conditions

  • Work performed in both clinical office settings and community environments (health fairs, outreach sites, partner locations).
  • Possible exposure to communicable diseases and bodily fluids in clinical environments.
  • Frequent interaction with a diverse population requiring sensitivity, empathy, and professionalism.
  • Regular use of computers, telephones, and office equipment.
  • Occasional evening, weekend, or extended hours for outreach or special events.
  • Must be able to travel between clinic locations and community sites as needed.
