SUMMARY OF POSITION:
The Outreach Coordinator plays an important and unique role in the Patient Experience outreach team. The Outreach Coordinator is responsible for recruitment and retention of patients. Primary responsibilities will include maintaining awareness of and building relationships with community partners and patient resources across the San Francisco Bay Area, particularly in or near neighborhoods where NEMS clinics are located. This person will develop and implement various outreach activities to NEMS’ current and future patient populations.
The candidate is also required to have efficient communication skills, able to build effective working relationships with patients and family members, healthcare providers and teams, and community resources providers. Strong communication skills will be used to improve coordination of patient care, reduce, and remove cultural and healthcare system barriers, promote timely treatment, empower, and coach patients to become self-advocates.

Essential Job Functions
- Outreaches to patients to schedule appointments for various clinical initiatives for all locations
- Contacts newly assigned patients to establish care at all sites
- Proactively communicates with patients to increase awareness of services offered by NEMS
- Understands the needs and interests of current and future patient populations
- Conducts outreach activities to diverse patient populations to generate interest in NEMS services and to promote program engagement and NEMS marketing strategy
- Seeks outreach and community awareness/presentation opportunities
- Assists in data collection and reports statistical information to Manager of Patient Experience
- Participates in Performance Improvement/Continuous Quality Improvement activities, and attends conferences and meetings as needed
- Ensures and remains in compliance with local, state, and federal regulations
- Performs other job duties as required by manager/supervisor
- High school diploma with two (2) years of relevant work experience, Bachelor’s degree preferred
- Medical appointment scheduling experience required
- MA/LVN or other clinical background preferred, understanding of medical terminology and clinical algorithms
- Experience in community development, social sciences, or health/science-related healthcare strongly preferred
- Strong English verbal and written communication skills
- Prior experience with customer service; can build rapport with patients via phone
- Proficient in scheduling medical appointments
- Familiarity with Epic or NextGen
- Ability to operate PC based software programs or automated database management systems preferred; proficient with Microsoft Office
- Good organization and prioritization skills, outstanding in time management
- Ability to follow through with delegated tasks and self-manage in a detail-oriented environment
- Fluent reading, writing, and speaking in English is required.
- Fluency in Spanish is required.
- This is an FLSA Non-exempt position.
- This is not an OSHA high-risk position.
- This is a Full Time position.
