We're seeking a Mental Health Case Manager to join the team and help make a meaningful impact on the lives of the clients. In this role, you'll conduct diagnostic evaluations, develop personalized treatment plans with specific goals, offer crisis intervention when necessary, and follow up with clients to ensure they receive the best care possible.

Qualifications

• A Bachelor’s degree in Social Work, Psychology, or a related field, or
• At least two years of experience in a direct service role (e.g., in an after-school program, nursing home, mental health setting, or client advocacy).
• 1-3 years of experience working with mental health patients.
