Licensed Occupational Therapist OT - Care Coordination

We are seeking a licensed Occupational Therapist (OT) to join a care coordination program supporting seniors in a variety of living settings, including independent, assisted, and memory care.

Your role involves providing occupational therapy and wellness services, conducting assessments, treatment planning, and therapeutic interventions as part of a multidisciplinary team.

You will support care coordination through regular wellness visits, communication with providers and caregivers, and assisting patients with telehealth visits.
