Job Overview

• Provide friendly, professional patient care, including episodic and chronic care, screenings, and education
• Work collaboratively with healthcare team, follow evidence-based practices, and participate in quality assurance and continuing education
• Perform vaccinations, screenings, and follow protocols, including after-hours on-call
• Refer complex cases and ensure proper documentation and follow-up
