Clinical & Quality Liaison - Registered Nurse

We are seeking a highly motivated and experienced Clinical & Quality Liaison RN to lead the coordination of home infusion nursing services for a specialty pharmacy serving patients across California. This hybrid role serves as the key liaison between the pharmacy and multiple contracted Home Health Agencies (HHAs) that provide skilled nursing care for home infusion services.
The ideal candidate will have a strong clinical background in infusion therapy, be detail-oriented, organized, and capable of managing multiple tasks in a fast-paced environment. This role requires proactive communication with providers, patients, HHA leadership, pharmacy staff, and internal clinical teams to ensure high-quality, compliant, and patient-centered care delivery. This position also plays a vital role in supporting accreditation and regulatory compliance.
Hybrid role based in Garden Grove, CA (10521 Garden Grove Blvd. Garden Grove, CA 92843) - onsite presence required for team collaboration, audits, training, and oversight one to two days a week. Flexibility to participate in virtual meetings, audits, and calls with remote teams and agency partners.
Pay Range: $90,000 - $105,000 based on years of experience

As the Clinical & Quality Liaison RN for our California Specialty Pharmacy you will…
- Serve as the primary clinical liaison between Advanced Infusion Care (AIC) pharmacy, Advanced Nursing Solutions (ANS) and multiple subcontracted nursing Home Health Agencies (HHAs) for the coordination of home infusion nursing services.
- Coordinates patient care with HHA, ordering physician, and care team to include pharmacist, clinical nurse liaisons, marketers, and reimbursement specialist.
- Collaborate closely with pharmacy leadership, scheduling teams, and internal nursing staff to ensure seamless service delivery and proactive patient care.
- Communicate regularly with patients, providers, and caregivers regarding nursing coordination, care expectations, and issue resolution.
- Conduct regular quality audits of nursing documentation submitted by HHAs to ensure completeness, accuracy, and compliance with accrediting body, state, and payer standards and applicable regulations.
- Performs audits on patient records to ensure compliance with HIPAA and accrediting body regulations and ensure survey readiness.
- Provide infusion therapy education, training, and clinical guidance to HHA nurses, as needed.
- Acts as liaison between pharmacy and home health agencies to ensure proper education of outside agencies and AIC policies are followed.
- In collaboration with Quality and compliance committee, is responsible for agency Process Improvement (PI) and infection control monitoring/reporting activities.
- Oversee nursing invoicing processes and ensure alignment with contractual agreements. Receive, review, process and reconcile HHA invoices, forward to Accounts Payable for payment.
- Review and manage contracts with HHAs, including onboarding, performance evaluation, and adherence to clinical standards.
- Prepare and present reports to the governing body related to clinical performance metrics, incident trends, audit findings, and compliance updates.
- Stay current with clinical best practices, accrediting body standards, and California nursing regulations related to home infusion therapy.
- Maintains knowledge of state specific regulatory guidelines that impact patient care.
- Active Registered Nurse (RN) license in the state of California, in good standing.
- Minimum 5 years of clinical nursing experience, with at least 2 years in home infusion, home health, or specialty pharmacy settings.
- Strong understanding of ACHC and URAC standards, home health operations, and infusion therapy protocols.
- Excellent communication skills with the ability to foster collaboration among pharmacy staff, nurses, patients, and external partners.
- Strong attention to detail, organizational skills, and the ability to multitask and problem-solve independently.
- Proficiency in EMRs, Microsoft Office Suite, and comfort working in a hybrid remote/in-office setting.
- Three years of nursing experience in clinical coordination, case management, nursing administration, or quality management preferred.
- Home Healthcare experience.
- Home Health Administrator experience.
- Quality and process improvement experience.
- Graduate of an accredited school of nursing required; Bachelor’s degree in Nursing (BSN) required.
