About the Job

The Criminal Justice Resource Coordinator serves as an OCHN Justice Initiatives team member embedded in the Oakland County Jail. The Criminal Justice Resource Coordinator acts as the primary OCHN contact to triage referrals for Eligibility Screenings, KITES and other OCHN service-related requests. This position represents OCHN, while coordinating between the provider network, Oakland County Jail, and the individual served to foster cross-system collaboration. The Criminal Justice Resource Coordinator ensures that individuals, prior to anticipated jail discharge, are provided with crucial documentation and information on specialty public mental health services through OCHN.

Essential Functions

Under general direction and as assigned:

  • Check for new admissions into the jail on daily basis. Contact aligned provider within one business day to inform of jail admission of individuals, who are actively receiving services within the OCHN MH provider network.
  • Manage the flow of referrals that are received via email, voicemail or in person.
  • Pick up KITES and triage accordingly.
  • Create a new medical record for individuals seeking an Eligibility Screening, or if applicable, complete initial documentation in medical record.
  • Obtain proof of guardianship papers, when applicable. Coordinate with guardians as needed.
  • Refer individuals to REACH Coordinator for Eligibility screenings.
  • Work closely with other OCHN liaisons, who intersect with justice partners pre-and/or post-release.
  • Connect out-of-county individuals with appropriate CMH contact.
  • Coordinate professional visits at the jail for the provider network in compliance with the jail’s visitation policy.
  • Attend all professional visits with individual and their service provider to assist with care coordination.
  • Coordinate with provider network to ensure that follow-up appointments are scheduled post jail discharge. Provide individual with appointment confirmation, address to provider and other pertinent information.
  • Request discharge prescription for individual. Ensure that individual’s prescription is provided upon release.
  • Point of contact for individuals post discharge if in need of prescription.
  • Facilitate discharge groups/1:1 meetings for individuals with an upcoming discharge date to determine individual need and educate on specialty public mental health services through OCHN, and other community resources. Communicate/Collaborate with treatment provider to ensure streamlined re-entry to the community.
  • Attend Behavioral Health Justice Collaborative and other required internal or external meetings
  • Collect data and share reports with OCHN leadership, alert of detected trends or changes, and share any identified barriers to achieving OCHN’s goals.
  • Assist in data-sharing effort between OCHN and justice partners.
  • Additional duties, as assigned.

Education

Job Requirements and Qualifications

  • Bachelor's Degree in Social Work , Psychology, or Criminal Justice .
  • Preferred: Master’s Degree in a mental health field .

Training Requirements (licenses, Programs, Or Certificates)

  • Preferred: State of Michigan licenses, certification and / or registrations as Psychologist (LLP/LP), Social Worker (LLMSW/LMSW), Counselor (LLPC/LPC), or Marriage and Family Therapist (LLMFT/LMFT ).

Experience Requirements

  • At least 2 years of experience providing service within the healthcare and/or criminal justice field.
  • Preferred: Experience in public mental health system.

  • Demonstrates high level of proficiency with Word, Excel, database, and other relevant software programs.
  • Demonstrates excellent interpersonal/customer service skills, both in direct personal interactions and on the telephone .
  • Demonstrates high level of oral and written communication skills.
  • Demonstrates ability to handle a high volume of relatively complex assignments accurately and timely .
  • Demonstrates organizational skills needed to establish office procedures and maintain files and records.
  • Ability to work independently in the field, while also functioning as a member of a larger team
  • Knowledge of the OCHN system, providers, eligibility criteria, and community referral resources.

Other Information

Special Information (Travel required, physical requirements, on-call schedules, and so on):

  • Must have available means of transportation to and from OCHN and for required offsite meetings or site visits.
  • Must be available for meetings and events which may occur outside of standard office hours.
  • Work performed primarily in an office environment.
  • Hybrid (onsite/remote) work schedule available.
  • Position is based primarily at the Oakland County Jail, 1200 N. Telegraph, Pontiac, MI 48340 .
  • Must possess and maintain a valid Michigan driver’s license.
