Job Description
Day to Day:
Plan, implement, and evaluate patient care based on established standards.
Interview patients to assess current health status and medical history.
Educate patients and families using instructional materials.
Provide psychosocial support and assist with locating community resources.
Manage medication refill requests and document in patient charts.
Schedule consultations, diagnostic tests, procedures, and follow-up appointments.
Triage patient phone calls and document relevant information.
Facilitate communication between patients, families, and care teams.
Act as a resource for LPNs, LVNs, and medical assistants.
Support interdisciplinary collaboration and clinical operations.
Adhere to HIPAA and company compliance standards.

Skills And Requirements
Must Haves:
Graduate of accredited nursing program; BSN preferred.
Active RN license and current BLS certification. null
