Duties and Responsibilities:
Comfort Home Care is seeking a Massachusetts licensed Registered Nurse / Home Health that can:
- Provide initial and ongoing assessments to identify the client's medical needs, level of functioning and ability to communicate and learn.
- Initiates the plan of care, necessary revisions, and performs OASIS assessments at appropriate time points when applicable.
- Initiates and performs appropriate nursing interventions.
- Coordinates services, informs the physician and other personnel of changes in the client's condition and needs.
- Collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
- Prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.
