Licensed Occupational Therapist OT - Care Coordination

A licensed Occupational Therapist (OT) is sought to join a senior care program supporting residents across independent, assisted living, and memory care settings.

• Provide occupational therapy and wellness services for older adults
• Conduct assessments, develop treatment plans, and perform therapeutic interventions within a multidisciplinary team
• Support care coordination through wellness visits, communication with providers and caregivers, and assisting with telehealth appointments
• Maintain professional and ethical standards
• Contribute to program development and quality improvement
