JOB DESCRIPTION:

Analyzing patients’ health status, goals and dietary restrictions
Creating individualized nutrition plans and meal plans
Educating and counseling patients and families on diet and nutrition
Tracking and assessing patients’ dietary intake and progress
Monitoring residents weight trends
Working with other healthcare professionals to coordinate care
Participation in inderdisciplinary meetings with clinical staff and department heads
Valid Registered Dietician for state of MA
