The Office of Diversity, Equity, Inclusion & Belonging (DEIB) at YoloCares is seeking a motivated and community-minded Program Coordinator to support the department’s daily operations and equity-focused initiatives.

Responsibilities
- Maintain existing business while achieving sustainable growth and market expansion
- Provide logistical and administrative support to the Diversity Manager, including scheduling, communications, budget tracking, and meeting preparation.
- Support the planning, execution, and evaluation of workshops, trainings, and public education events focused on health equity in grief support, hospice, and palliative care.
- Cultivate relationships with local community support focused programs for outreach, event collaboration, and referral partnerships.
- Assist in implementing community-informed recommendations from YoloCares led community needs assessments, such as the Life Transitions Project and other end-of-life care focused assessments.
- Represent the office at tabling events, community pop-ups, and cultural gatherings (some weekends/evenings required).
- Create and maintain resource directories tailored to Indigenous, rural, and underserved populations.
- Assist in tracking impact metrics, DEIB benchmarks, and SDOH indicators that guide department interventions and reporting.
- Collaborate across departments to ensure inclusive and culturally responsive communication and service provision.
- Stay informed of relevant policies and trends (e.g., CalAIM, Medicare hospice updates, IHS policy) to support culturally competent care planning.
- Perform other duties and special projects as assigned.
- Bachelor’s degree or equivalent experience in a relevant field (e.g., public health, social work, education, or sociology)
- 2+ years of experience in program coordination, community outreach, or DEIB-related work
- Demonstrated cultural humility and understanding of the historical and contemporary contexts impacting Indigenous and/or underserved communities
- Strong communication and relationship-building skills across cultural and institutional lines
- Proficiency in Microsoft Office and virtual collaboration platforms (Zoom, Teams)
- Lived experience or long-standing relationships in Indigenous, rural, or underserved communities served by YoloCares
- Familiarity with Medicare, IHS, Tribal health, or CalAIM
- Experience with workshop planning, coalition-building, or cross-agency coordination
- Spanish, or other community language proficiency
- Familiarity with grief support, hospice, or public health education framework
- Able to hear, speak, read, write, and type well enough to perform required job duties.
- Able to lift up to 35 pounds without physical problems.
- Must have a valid California driver’s license and state-required automobile insurance if driving on Yolo Hospice, Inc. business.
- Must be willing and able to flex hours to meet program needs on evenings and weekends.
