Job Responsibilities

• Deliver direct care to rehab patients recovering from surgery, trauma, or chronic illness
• Monitor patient conditions and update care plans accordingly
• Perform wound care, medication administration, and mobility assistance
• Collaborate with the interdisciplinary team to optimize patient recovery outcomes
• Maintain precise and timely documentation of all patient interactions
• Support patient education and discharge planning
  
  

Qualifications And Certifications

• Current RN license valid in Oregon or compact state eligibility
• BLS certification required
• 1+ years of rehab, med-surg, or post-acute nursing experience preferred
• Excellent patient interaction and team communication skills
