Our goal is simple: improving access to healthcare in our community and surrounding areas. Neighborhood Health is looking to add a Pharmacy Manager to our team, reporting directly to the Senior Director of Pharmacy. Together, we

As a Pharmacy Manager, you will:

✓ Oversees the day-to-day operations of the Pharmacy department.

✓ Conducts and evaluates medication histories, assesses compliance and suggests modifications for optimal drug

product selection in order to achieve desired outcomes; instructs patients on correct use of prescribed

medications.

✓ Individualizes medication regimens using sound principles and understanding of drug absorption, distribution,

metabolism, and elimination to establish and improve to optimum patient benefit.

✓ Effectively verifies patient medication orders for appropriateness and accuracy. Provides medication

reconciliation and evaluates medication appropriateness and completeness with respect to patient data,

including age, height, weight, diagnosis, and other pertinent patient information.

✓ Prevents, detects, and resolves medication-related problems. Makes evidence-based, patient-centered drug

therapy recommendations as part of the Integrated Care Team in a PCMH Model.

✓ Will help lead and assist the organization’s 340B oversight team in conjunction with the pharmacy director,

which will include representation from pharmacy, legal, compliance, finance, and senior administration.
