Serves as an integral member of the interdisciplinary team for the Integrated Health Home Initiative. Functions collaboratively with the Integrated Health Home Team and the other disciplines to provide and enhance care coordination. Adheres to all nursing policies and ethical standards, and proactively strives to maintain and improve quality outcomes.
Key Accountability: Care Coordination• Monitor the high-risk care coordination plan and facilitate care and communication amongst treating behavioral and physical providers.• Collaborate with Health Home Team and treating providers in developing and implementing care coordination plan to address treatment barriers for each high-risk member.• Conduct home visits, attend meetings, staffings, care plan conferences, etc. related to enrolled members.Key Accountability: Patient Care• Promote patient's health and ensure that all personal health goals are included in person-centered care management plans.• Develop symptom response and crisis plans, ensuring more appropriate use of physician visits rather than emergency room and hospital admissions.Key Accountability: Patient Education• Provide health education to patients about preventing and managing chronic conditions using evidence-based sources. • Provide self-management support and development of self-management plans and/or relapse prevention plans so that members can attain personal health goals.Key Accountability: Clinical Excellence• Documentation of services using the Center's Electronic Health Record and other Provider Portals as required.• Maintain patient confidence and protect operations by keeping information confidential. • Assist with the collection of data for special projects and presentations for Integrated Health Home program.• Adhere to all policies and procedures set forth by UnityPoint Health and other funding sources.Basic UPH Performance Criteria • Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines.• Demonstrates ability to meet business needs of department with regular, reliable attendance.• Employee maintains current licenses and/or certifications required for the position.• Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.• Completes all annual education and competency requirements within the calendar year.• Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.
Graduate of accredited Nursing Program with a license within the State of Iowa.
Current BLS; Valid Driver's License.
